[Cuis-dev] Rename Instance Variable improvement

Hi,
 attached is a .cs that enhances the rename instance variable refactoring
in many ways:
1) It works correctly when renaming in the debugger
2) Class instance variables can be renamed from the metaclass definition in
the browser
3) It works faster :-)

 It also includes a refactoring to make well know category names be in one
place + when creating a method from the debugger and the receiver is a
class, it puts the category 'instance creation' below 'new...' because most
of the time that is what you need and it was not easy to access to it.
